Please print and mail the Submission Sheet when sending samples to the lab. Please fill out the submission sheet with as much detail as possible.
On January 1st, 2026, the price for a plant insect pest diagnostic will be $30 per sample.

Suspected Herbicide Damage
We cannot confirm herbicide damage. Please visit the Tennessee Department of Agriculture if you suspect you are dealing with a commercial misapplication of herbicides.
